Position Summary

• Coordinates the integration of Social Services functions into the patient care, discharge and home planning processes with other hospital departments, external service organizations, agencies and health care facilities. 
• Ensures compliance with regulatory standards to social service activities. 
• Utilizes knowledge of population appropriate guidelines to care for the infant, pediatric, adolescent, adult and geriatric patient.

• Master's Degree of Social work required. 
• Must be certified to practice as a Licensed Social Worker in the state of South Carolina. 
• Significant experience required as a Social Worker, preferably in a health care environment. 
• Must have effective interpersonal skills for working with patients and their family members.
• Demonstrated oral and written communication skills required to communicate in a clear and concise manner with patients and families, physicians, staff, public, administration and outside agencies.
