Position Summary
The MSW Case Manager serves as a key leader in the complex discharge planning process and plays a vital role on the interdisciplinary healthcare team. This role involves assessing, planning, implementing, and evaluating safe and effective discharge plans, particularly for patients with psychosocial complexities. The MSW Case Manager collaborates with RN Case Managers, physicians, and external agencies to coordinate care that aligns with the patient’s needs and available resources. This position requires strong communication, critical thinking, and documentation skills to ensure seamless care transitions and optimal patient outcomes.
Essential Functions
• Assessment: Evaluate patients' post-acute care needs including functional status, cognitive abilities, support systems, available resources, and barriers to care
• Planning: Collaborate with the patient, decision-makers, and interdisciplinary team to create proactive, individualized discharge plans
• Communication & Documentation: Clearly communicate discharge plans to the care team and external providers. Document assessments, care plans, and interventions in the medical record
• Evaluation: Monitor progress toward discharge goals and modify plans based on patient needs and outcomes
• Delegation: Appropriately delegate tasks to ensure timely and safe execution of discharge plans while maintaining patient safety
• Professional Accountability: Uphold ethical standards, maintain confidentiality, and advocate for patient rights
• Teamwork: Build positive relationships with internal and external stakeholders. Support peers through mentoring, education, and conflict resolution
• Resource: Serve as a consultant for discharge planning in psychosocially complex cases and maintain knowledge of community and post-acute resources
Qualifications
• Master’s degree in Social Work (MSW) from a program accredited by the Council on Social Work Education — Required
Experience
• Minimum of 2 years of professional social work or healthcare experience — Preferred
• Prior case management or care coordination experience — Preferred
Skills & Requirements
• Availability for weekend and holiday coverage — Required
• Strong knowledge of Microsoft Office and electronic documentation systems — Preferred
• Ability to manage psychosocially complex discharge plans and work in a multidisciplinary environment
